And. The Kamianets-Podilskyi Lyceum with enhanced military and physical training of the Khmelnytsky region accepts boys and girls (girls without 24-hour (night) residence),) - citizens of Ukraine aged 14-16, who are able according to their condition health to study at a lyceum, expressed a desire to become officers of the Armed Forces and other military formations of Ukraine and graduated from 8th grade in 2024.

II. Boys and girls (girls without round-the-clock (night) residence) who have expressed a desire to enter the lyceum, apply by April 1 until June 10, 2024 > such documents:

1. A personal statement addressed to the head of the lyceum about the desire to study at the lyceum and readiness after graduation to continue studying at a higher military educational institution or an institution of higher education of other power structures of the security and defense sector of Ukraine
2. Statement of parents (or persons replacing them), indicating their consent for the education of a young man or a girl at a lyceum.
3. Consent of parents (or persons replacing them) to the processing of personal data of their minor child
4. Passport and 3 photocopies of it (if you are 14 years old at the time of admission), AND a copy of the birth certificate (if you do not have a passport, then 3 photocopies);
5. Copies of the taxpayer's card (identification code) - 3 pcs.
6. Certificate on the composition of the family (in accordance with the new administrative division of districts and territorial communities)
7. Characteristics and report card for the 8th grade (for the first semester), certified by the signature of the director and the seal of the institution of general secondary education
8. Copies of certificates and diplomas certifying victory in Olympiads and sports competitions, letters of commendation issued for academic success (for the last year of study).
9. Photo cards size 3 x 4 cm without headdress - 4 pcs.
10. Medical certificate form No. 086/о , to it is added:

-    ECG with decoding;

- fluorography (description) (from the age of 15!);

-     extensive blood analysis;

-     blood sugar test;

-     general analysis of urine;

-     enclosure letter for a teenager to the medical card (form No. 025-1/0 from the age of 14);

-     card of preventive vaccinations from the date of birth to 2024 (form No. 063/о) - 2 copies;

- history of child development (ambulatory card) form 112/o.

Entry tests are held between June 11 and June 14 u>2024.

III. Entrance tests include:

1. Professional entrance tests:

-     psychological examination (tests);

 -    medical examination;

- level of physical fitness (pull-ups, running 60m, cross country 1000m).

2. An interview is held for 8th grade graduates

(Ukrainian language, mathematics, physics, history, foreign language, biology).

IV. The right to enroll outside the competition, provided that they successfully pass the entrance exams, have orphans and children deprived of parental care; children of combatants who died while performing official duties.

The right to priority enrollment, subject to successfully passing the entrance exams, have:

children of military personnel and combatants; children from large families; children who suffered themselves or their parents from the Chernobyl disaster and belong to categories 1-2.

Accommodation and food for entrants during the period of entrance exams is paid for by parents.

The Admissions Office works from Monday to Friday from 9.00 to 16.00, lunch break from 13.00 to 14.00

II. Boys and girls (girls without round-the-clock (night) accommodation), who expressed a desire to enter the lyceum, submit to term from April 1 to June 10, 2024 > the following documents:

1. A personal statement addressed to the head of the lyceum about the desire to study at the lyceum and readiness after graduation to continue studying at a higher military educational institution or an institution of higher education of other power structures of the security and defense sector of Ukraine
2. Statement of parents (or persons replacing them), indicating their consent for the education of a young man or a girl at a lyceum.
3. Consent of parents (or persons replacing them) to the processing of personal data of their minor child
4. Passport and 3 photocopies of it (if you are 14 years old at the time of admission), AND a copy of the birth certificate (if you do not have a passport, then 3 photocopies);

8. Certificate of basic general secondary education (entrants submit to the lyceum admissions committee in person upon arrival for entrance exams).
9. Copies of certificates and diplomas certifying victory in Olympiads and sports competitions, letters of commendation issued for academic success (for the last year of study).
10. Photo cards size 3 x 4 cm without headdress - 4 pcs.
11. Medical certificate form No. 086/o, to which the following is added:

-    ECG with interpretation;

- fluorography (description) (from the age of 15!);

-     extensive blood analysis;

- blood sugar test;

-     general analysis of urine,

-     enclosure letter for a teenager to the medical card (form No. 025-1/0 from the age of 14);

-     card of preventive vaccinations from the date of birth to 2024 (form No. 063/о) - 2 copies;

- history of child development (ambulatory card) form 112/o.

Entrance tests are held in the period from June 17 to June 21 >June 2024.

III. Entrance tests include:

1. Professional entrance tests:

-     psychological examination (tests);

 -    medical examination;

- level of physical fitness (pull-up, 60m run, 2000m cross country).

2. Entrance exams for general education subjects:

-     Ukrainian language (dictation and grammar task);

-      mathematics (written).

AndV. Orphans and children deprived ofparental care; children of combatants who died while performing official duties.

The right to priority enrollment, subject to successfully passing the entrance exams, have:

children of military personnel and combatants; children from large families; children who suffered themselves or their parents from the Chernobyl disaster and belong to categories 1-2.

Copies of documents certifying the right to priority admission or non-competitive admission to the lyceum are submitted together with other documents.

Accommodation and food for entrants during the period of entrance exams is paid for by parents.

The Admissions Office works from Monday to Friday from 9:00 a.m. to 4:00 p.m., lunch break from 1:00 p.m. to 2:00 p.m.
